11/1/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, client appeared to be in a content mood evident by his facial expressions such as a smile on the face. QMHP-C engaged mother in a discussion to determine his behaviors since the last session. Mother verbalized to QMHP- C on how client struggles with getting along with his peers as well as his siblings. Behavior: During second portion of the session, client was observed by QMHP-C arguing with is sibling as QMHP-C and mother was discussing with client ways to get along with his sibling as well as his peers within the school, home and community setting. Intervention: QMHP-C facilitated activity to allow Client insight into several conflict resolution and problem-solving skills to utilize at home. QMHP-C encouraged Client to role play appropriate and inappropriate conflict resolution skills in efforts to highlight and demonstrate several appropriate options Client may utilize when he feels challenged or the need for impulsive disruptive actions. QMHP-C provided strategies such as positive reframing. QMHP-C assisted Client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations that usually trigger Client's negative expressions. Client was able to reduce physical overactivity and become more attentive in session. Intervention: QMHP-C discussed with the family one goal they believe they need to work on individually and as a family. QMHP-C had each member write down goals and discussed through brainstorming and creating a goals chart on how to achieve each one. QMHP-C prompted Client to express how he believes his family should treat each other. QMHP-C encouraged Client to think twice before reacting negatively or aggressively in situations that may not need that level of reaction to be solved. QMHP-C used the basic listening sequence to clearly draw out the point of view that Client was feeling and get beyond his aggressive language and behaviors. Response: Client understands that problem solving starts with thinking before acting. Client demonstrates problem solving by staying calm and using his senses. Client acknowledges expectations and reviews goals and objectives with counselor and parent in order to get familiar with how he is supposed to react to different conflicts in the home. Client displays good social behavior, as seen sharing and taking turns. Client implemented the positive reinforcement provided by his counselor and was able to stay motivated and enjoy his day. Client exhibited good manners after earning incentive for his hard work. Response: Client focused on things he is good at and loves to do at home in order to stay motivated in the home/community setting and make something out of himself. Client did a good job ignoring distractions and maintaining appropriate behavior in the home. Client administered positive behavioral expectations, reducing ADHD. As Client has made more of his own decisions, he reported increased satisfaction and function. Client responded well to feedback, interacting well with his peers. Client agreed to behave and communicated a positive attitude when reviewing problem solving and self-control strategies. Client promised he would try his best to correct himself when faced with conflict.
